Patient Eligibility

1.
Terminal Prognosis
n
Primary and hospice physician must both certify terminal prognosis of six months or less
n
Patient should be exhibiting a progressive decline
n
Consider the whole picture of the patient’s condition, not just the diagnosis
   
2.
Desire hospice
n
Patient/family fully understand terminal prognosis
n
Patient/family fully understand their options and choose to seek palliative care rather than aggressive therapies
n
Patient/family fully understand Hospice philosophy
   
3.
Insurance/Coverage
n
Medicare patient must be eligible for Part A and not be utilizing Medicare for other therapies (skilled)
n
Medicaid - same as Medicare
n
Private/HMO - most have Hospice coverage
n
Financial Assistance - Hospice & HomeCare By The Sea, a not-for-profit agency, provides hospice
n
care regardless of age, race, or diagnosis and with generous financial assistance to the needy in our community.

n
Adult Living Facilities and Skilled Nursing Facilities
n
Hospice By The Sea Inpatient Care Center or Other Contracted Acute Facilities

Hospice & HomeCare By The Sea will assess patient within 24 hours to determine appropriateness and level of care
