When a referral is
made, the hospice representative comes to visit you wherever you
are. Hospice philosophies, services, and the available options
are explained. When the hospice choice is made, an informed consent
and other appropriate documents are signed to begin hospice care.
You will be given a patient and family information book as a guide
and resource. The representative will review it with you and answer
any questions that you may have.
Once the level of care
is determined as a hospice patient, there will be a team of people
that will help take care of you. They will include:
- the patient's personal
physician
- hospice physicians
- nurses
- certified nursing
assistants
- pharmacist
- social workers
- therapists
- volunteers
- spiritual care counselors
- bereavement counselors
The interdisciplinary
team will perform a physical, psychosocial, and spiritual assessment
to determine the individual plan of care. The focus of the care
plan is comfort.
Keep in mind that hospice
is a choice. You can choose to discontinue hospice services at
any time. You will always be welcomed back if you ever decide
to resume services.
