POSITIONS WITH BI-LINUGAL TEAM

BI-LINGUAL TEAM – CNA – FT
The Bilingual Team Certified Nurse Assistant/Home Health Aide is a non-professional with training in the supportive services, which are required to provide and maintain bodily and emotional comfort.

POSITIONS WITH THE FACILITY TEAM

FACILITY TEAM – RN – FT
The Registered Nurse is responsible for providing skilled and highly technical direct patient care and supportive services to the patient/family unit. The care and services provided follow the individualized patient/family care plan developed by the Interdisciplinary Team. He/she reports to the Team Coordinator concerning the maintenance of palliation.

POSITIONS WITH HEALTH INFORMATION MANAGEMENT

HEALTH INFORMATION MGMT – DATA ENTRY/AUDITOR – FT
The Auditor/Data Entry person provides support to the patient care staff by performing censuses, reports and other computer services that provide Health Information to the Patient Care Teams. The Auditor/Data Entry person follows the established HBTS Health Information Procedures and reports areas of concern to appropriate staff for follow-up. The Auditor/Data Entry person is also trained to provide support and back-up for other members of the Health Information Department.

HEALTH INFORMATION MGMT – SPECIALIST HC - FT
This position performs Clinical Data Entry and also supports the patient care staff by compiling censuses, routine reports and providing other computer services, which distribute Health Information to the HomeCare staff. This position provides support to administration by providing special reports as needed, supervising special H.I.M. related projects and participating in management activities. The Health Information Specialist is also trained to provide support and back-up for the other members of the health information department. The Health Information Specialist, is a self-motivated, detail oriented, person with the ability to work independently on complex projects.

POSITIONS WITH HOME TEAM

HOME/FACILITY TEAM – ASSISTANT – FT
The Home & Facility Teams Assistant is responsible for the overall coordination of the clerical operations of the teams. The assistant directs the flow of phone calls that come into the department. His/her skills allows him/her to audit the field staff itineraries for supervisory approval, update and generate patient care plans and process the paperwork trail within the department. The assistant position also requires having experience with payroll and has the ability to maintain confidentiality.

HOME TEAMS – RN – NORTH – FT
The Registered Nurse is responsible for providing skilled and highly technical direct patient care and supportive services to the patient/family unit. The care and services provided follow the individualized patient/family care plan developed by the Interdisciplinary Team. He/she reports to the Team Coordinator concerning the maintenance of palliation.


HOME TEAMS – TEAM LEADER – NORTH – FT
The Home Team Leader is a professional nurse responsible for the coordination of the Home Inter-disciplinary Team. He/she assures that the plan of care is developed and continually revised to deliver hospice services to the patient/family unit. He/she supervises direct patient care provided by the team assuring the physical, emotional, spiritual and financial needs of the patient/family unit are addressed.

POSITIONS WITH HOMECARE

HOMECARE – STAFFING COORDINATOR – FT
The HomeCare Staffing Coordinator is a non-professional with training in the office support systems and excellent communication skills. Staffing responsibilities will include: scheduling all disciplines who provide patient care; answering the phone, communicating with staff, patients and vendors; data entry; filing; maintaining accurate scheduling records; on-call scheduling; processing invoices; medicare/medicaid/ insurance verification and other routine office tasks.

HOMECARE – HI-TECH – RN – FT
The registered nurse is responsible for providing skilled and highly technical direct client care and supportive services to the client/family unit. The care and services provided follow the individualized client /family care plan developed by the Interdisciplinary Team. He/she reports to the Director of Nursing concerning the client's condition.

HOMECARE – HIGH-TECH - LPN – FT - NORTH
The Licensed Practical Nurse shall provide selected nursing care including the administration of treatments and medications to the patient under the direction of a registered nurse. He/she shall schedule home health aide coverage and take calls to assure that coverage. In addition, office duties include answering the phone, maintaining patient records filing and routine office tasks. Homecare and Medicare experience preferred.

HOMECARE – HI-TECH – RN – FT

HOSPICE STAFFING SERVICES – CNA – DAYS – FT
3 POSITIONS
The Certified Nursing Assistant is a non-licensed person with training in supportive healthcare services. He/she provides services that are ordered by the physician in the plan of care and that the Certified Nursing Assistant is permitted to perform under state law. The duties of the Certified Nursing Assistant include the provision for hands-on personal care, assistance with activities of daily living and to offer simple emotional support to clients receiving Hospice services in a private home residence.

HOSPICE STAFFING SERVICES – LPN – PER DIEM
The Licensed Practical Nurse is responsible for providing skilled nursing care to both hospice home and facility clients under the direction of a registered nurse. The care and services provided follow the written plan of care developed by the Interdisciplinary Team.

PATIENT ACCOUNTS – ASSISTANT – FT
The Patient Accounts Assistant is a highly responsible, self-motivated individual with training and experience in Medicare, Medicaid, and commercial insurance billing, and training in the office support systems, which are required to provide and maintain the smooth and timely communication of information. The Patient Accounts Assistant responsibilities include verification of Medicare, Medicaid, and commercial insurance coverage, negotiating coverage with case management representatives, data entry of patient care services, generation of billing, and collection of aged accounts.

POSITIONS WITH PROFESSIONAL RELATIONS

PROFESSIONAL RELATIONS – LIAISON – FT
The Professional Relations Liaison assists the Director of Professional Relations with all aspects of marketing Hospice/HomeCare By The Sea services to the professional community, including: physicians, healthcare professionals, hospitals, nursing facilities, assisted living facilities and various community organizations.

POSITIONS WITH REFERRAL AND ADMISSIONS

REFERRAL CENTER – ADMISSIONS PROGRAM COUNSELOR – FT
The Admission Program Counselor is responsible for responding to referrals for Hospice and HomeCare services as directed by the Director of Referral Center. The Admissions Program Counselor will meet with the patient/family to discuss the Hospice and HomeCare program and obtain consents for Hospice and HomeCare services. The Home Program Counselor will provide general information regarding Hospice and HomeCare services to referring institutions and agencies. He/or she will report to the Director of Referral Center concerning the admission status of the patient on a daily basis. Must have bachelor’s degree in health related or human services field (i.e. Social Work, Psychology). Two years or professional experience, preferably in the health field.
